The funeral service will be on Saturday, April 27, 2019 at 1:00 p.m. at St. Mark Missionary Baptist Church, 2703 Forman Circle, Middleburg, Florida 32068. Interment will follow at Grant Forman Cemetery, 4441 Witch Hazel Road, Middleburg, Florida 32068.
